Principal/Special Education Director

Re: Request for Special Education Evaluation —

Dear Principal/Special Education Director,

I am writing to request a comprehensive evaluation of my child, , who is currently in grade and is years old. I am concerned that may have a learning disability, specifically dyslexia, that is affecting their ability to make progress in school.

I have observed the following difficulties:

• Reading and learning difficulties

Under the Individuals with Disabilities Education Act (IDEA), I am formally requesting that the school conduct a full and individual evaluation at no cost to our family. This evaluation should include assessments for reading disabilities, language processing, and any other areas that may be affecting 's academic performance.

Please respond in writing within 10 business days with either a copy of the evaluation plan for my consent, or a written notice explaining why the school refuses to conduct the evaluation.

I look forward to working collaboratively with the school team to support 's learning needs.

Sincerely,

Parent/Guardian of
